This commitment to excellence and proven outcomes supports Senior Reach in strengthening program activities, ensuring fidelity, demonstrating positive results, and, ultimately, ensuring the highest quality of care for seniors.

The Senior Reach Annual Formal Review is the established mechanism to provide feedback to communities implementing Senior Reach in order to maintain quality and prove fidelity, as well as to satisfy stakeholders, consumers and family members. Several key measurements have been developed to address performance and are related to the Six Purposes of Senior Reach, the goal being for programs to achieve an 80% aggregate score.
