Senior Reach trains the community to identify and refer seniors in need. The Outreach and Education is the vehicle to do this. Explore and engage agencies and groups in your community that could host community partner trainings to community members. Identify community members who would be ideal Community Partners such as health and human service providers, neighbourhood associations, postal carriers, religious communities, etc. Licencees will receive all necessary training, materials and consultation to train Community Partners.
Explore and engage call center models already active in your community, this could be through a senior center, human service provider, or 211. Licencees will receive all necessary training, materials and consultation to develop the Call Center.
Explore and engage providers in your community who deliver essential Senior Reach services including behavioral health, care management and other critical community based services. Focus on organizations receiving Area on Aging funds and consider Medicare billing processes. Licencees will receive all necessary training, materials and consultation to develop Services.
